Records of psychiatric or mental illness diagnosis, treatment, and prognosis
Documentation related to any harmful or violent behavior, including cause (psychiatric, medical, drug, or alcohol-related)

Disqualification
Applicants may fail the immigration medical exam if diagnosed with certain communicable diseases, including:
- Tuberculosis
- Syphilis
- Gonorrhea
- Leprosy
- Quarantinable diseases listed by U.S. Executive Order
- Public health emergencies as reported to the World Health Organization (WHO)

Validity of Exam Results
After your exam is completed, you will receive your results for submission to USCIS.
Your results are valid for two years from the signature date on the I-693 form.
